Where can lvl 4 syn stones be found?!!

I must have played stage 4 reward levels at least 100 times each.. never seen a lvl 4 syn stone before

Base of feedback from my server's players.. I conclude that Lvl 4 syn stone can only be found on 4-20 or 4-45 or crystal boxes.. But they are as rare as getting Angelo -.-
