Coin buying problem

Although my itunes account has enough money, the buy coins feature, whenever I press buy, continuously shows a message saying "Purchase failed or cancelled". Can anyone tell me a reason why, or what to do?

IOS version 6.1.3, Iphone 4, language is set to English. However, I also tried this on an IOS 5.something itouch, and both my parent's phones, and its still not working.

I made a purchase of 270 gold coins, but to me they were never delivered. coo card accounts written off the money, but the purchase, I never got it. It's been over 5 days. what to do?
