How to report an Android bug

There are a lot of Android phones with different firmwares (which are for example Gingerbread 2.3.5 or Froyo 2.2.2 etc.)
To help developers understand the problems you should report:
1) The phone
2) The firmware
3) The rom (Cyano, MIUI etc. or the stock one)
4) What problems are you experiencing
5) When are you experiencing them

For example:

I'm using a Samsung Galaxy S2 with Gingerbread 2.3.4, I'm using the stock rom.
After I entered the name and the password and I clicked ok the game force closes instantly, it doesn't even show loading.
